Staying at 300mg Abilify

In the end, I didn't lower the intramuscular Abilify injection this month; I stayed at 300 mg of Maintena. However, I saw that I could perhaps lower it to 200 mg and then 160 mg. Next time I talk to my psychiatrist, I'll ask him to lower it to 200 mg, since he was a bit reluctant to switch to the 15 mg oral tablets. Perhaps this could be a good solution, and even quicker than tapering off the pills. What do you think?

It's not possible to taper off abilify using injections. You'll need pills / liquid abilify or water suspension for safe tapering.

You have been on abilify for a long time, so please don't try to rush your taper. Doing it too quickly can be dangerous for your health.

@guava378 I must have explained myself poorly. My plan is to go to 200 mg, then maybe 160 mg, and then switch to the tablets, the equivalent of 160 mg. I'm aware of the hyperbolic nature of the reduction, and I know abrupt withdrawal can cause serious damage. To be clear, I don't intend to do that.

Hi @DiSoleEDAzzurro. I did lower from Abilify Maintena. First 400, then 300mg, 3/4 ampoule and then 1/2 ampoule. In between every drop there was at least 1 month hold. My next step was to then switch to the oral pills. In this case, as I found out, 150mg was equal to 7,5mg. From then onwards, I began making water suspensions and lowering in hyperbolic fashion. I am currently on 0.70mg. Note that I did the switch to oral just half a year into the treatment with the injection.

Thanks so much for the comment @Vilkvinn t's really valuable, but I have a few questions for you. How did you feel during this reduction journey? Did you have any difficult moments? Were you also afraid of withdrawal symptoms? One more thing: by 3/4 ampoule and 1/2 ampoule, what exactly do you mean? In terms of milligrams? How long did it take you personally to go from 7.5 mg to where you are now? Thank you so much if you could answer these questions.

@guava378 He's not totally convinced of me tapering off the medicine, he proposed me (the last time I went there) to go every day for the capsules to be taken, which is crazy if you think about it, probably because he do not trust me taking the pills. I do not fully understand if he is supportive or not

@guava378 They got me psychosis and schizofrenia label when I stopped suddenly my 5mg abilify and 75mg efexor in december 2025. So hospitalization followed.. I have also isolation tendency and i don't really know how to fix that outside medicines effects on me

@DiSoleEDAzzurro overall, the only time I felt very affected by the reduction was when I tried to rush my taper (I wanted to jump from 2mg to 1,5mg) and I experienced debilitating insomnia. I fixed it by going back up to 1,8mg and taking it much slower. There were times when I also experienced the effect of the reduction but in a milder way (a few nights of reduced sleep time or light anxiety) when I jumped from the whole vial of 300mg to 3/4 and then to 1/2*. Nothing that lasted long or became concerning. Also, I do remember that the first week of taking the pills, there was some discomfort, mainly with digestion and feeling a bit on the edge, agitated, as if there was some overlapping of potency between the effect of the depot from previous months and that of the pills. Somehow I expected it because I had read somewhere that the intensity the first days would be strong so I kept going. As the days went on, the discomfort subdued. But I kept checking in internally every day how I was feeling and if there was a sign of upcoming psychosis, I would immediately jump up to 10 or 15 mg. So, I'd say, it has been a smooth journey, except when trying to speed up the taper. Regarding how I have been feeling towards the whole process, apprehensive would be the word. The more information that came to me the more the awareness of a possible development of dependency grew. However, as it proves from my mistakes, I needed to have 6 days of very little sleep to realise that it was a matter of more patience and taking it month by month. In August it will mark the first year on pills. Previous to that, I had been 8 months on injections and the reductions were subjected to the appointments I had with my psychiatrist since she was the one to modify the prescription. So sometimes the hold would be 2 months, sometimes 1. The last cut to half the vial, in june, lasted 2 months. Right from the beginning, something stuck with me: the psychologist I was seeing back then told me that there were people he knew who 15 days after the due depot, had to be hospitalised. That gave me the urge to closely monitor what I was doing to not to mess things up. The schedule was something like this: 2 months and half at 7.5mg by halving the 15mg pills. Then water solution and reduction to 5mg, hold for 1 month. Then cut to 2.5mg, hold for a month. Another cut and hold of 2mg. In my mind, I know recall trying to replicate what Zlatinb and Salaam were doing as I had read on the Surviving Antidepressant forum: big cuts in the higher range and then slowing down once I hit below 2mg. Somehow it worked. Next were 1.80mg, 1.65 mg, 1.45mg, 1.20mg, 1.00mg, 0.85mg, and now 0.70mg. This is no recipe for anyone. I am not the most knowledgeable when it comes to tapering. It has been mostly trial and error. *that was the instruction my doc gave in the prescription to the nurse. I know it is a bit of rough specification because it is done by the nurse by eye, but that's how they did it. To me 150mg was the equivalent to half the vial.
